Be Healthy

Getting regular preventive care and check-ups is the cornerstone to good health. We cover 100 percent of charges for an annual examination (in-network for PPO members) and medically appropriate screenings. For a list of covered screenings, please see the Medicare version of our Using Your Preventive Care Benefits brochure.

Personal Health Coordination

Health Alliance Medicare PPO and HMO plan members who are eligible have access to nurse case managers who help make coping with hospital stays and chronic illnesses easier. Nurse case managers can:

  • Explain your diagnosis and test results in terms you understand.
  • Discuss your treatment options and what you can do to improve your health.
  • Take the confusion out of hospital discharge notices and other documents.
  • Assist with home health care, skilled nursing facility stays and durable medical equipment.

Be Fit
