PPO 30
Our PPO plans—available in 41 Illinois counties—allow you to see any doctor, but you’ll pay less when you see the host of doctors in our network.
Plan Features
- The monthly premium for PPO 30 is $45.
- PPO 30 has lower monthly premiums than HMO 20 (HMO) and PPO 10 (PPO).
- Coverage for in-network services without having to meet a deductible.
- You won’t pay more than $3,000 per year for covered in-network services.

Add Prescription Coverage
You can add prescription coverage to PPO 30. The monthly premium with it is $89 per month.
