Enroll Online
You can enroll online through the Centers for Medicare & Medicaid Services website.
Enroll through the Mail
Or you can download our HMO and PPO enrollment form or our Stand-Alone Prescription Drug Plan enrollment form. Also, our Medicare Supplement enrollment form.

Are You Eligible?
If you’re interested in a Medicare Advantage plan, remember there are only certain times during the year when you can enroll. See When You Can Enroll for more information.
To be eligible for a Health Alliance Medicare Advantage plan
- You must have Medicare Part A and Part B and live in the service area. (See Our Plans for maps of our HMO and PPO service areas.)
- You must continue to pay your Medicare Part B premium, if not otherwise paid for under Medicare or by another third party.
- You cannot have end-stage renal disease (permanent kidney failure).
With an HMO plan, if you obtain care from out-of-network providers, neither Medicare nor Health Alliance Medicare will be responsible for the costs. See Enrollment and Disenrollment for specific requirements for our Medicare Advantage HMO and PPO plans.
To be eligible for a Medicare Supplement plan
If you live in Illinois, have Medicare Parts A and B and are at least 65, you are eligible for a Health Alliance Medicare Supplement plan.
After You Enroll
We don’t stop interacting with you when your enrollment form arrives. We’ll help you get settled in with your Health Alliance Medicare Advantage plan.
- We’ll call to verify that your coverage is in place and talk with you about how to make the most of your benefits.
- We’ll mail your member materials, including our helpful member handbook.
- We’ll send your ID card for use at doctor, hospital and pharmacy visits. You’ll use this card instead of your red, white and blue Medicare card.
